The forms and brochures posted here are generic. You will need to contact your employer to obtain the enrollment and change forms specific to your benefit program. Your employer has provided you with a copy of the Summary Plan Description (SPD). Please refer to your SPD for all eligibility, enrollment, claims filing deadline and other specific issues.

Claims created on the website and received by fax, mail or our new secured web claims portal by 9am Pacific on regular business days are processed same-day for the fastest possible reimbursement.
