The benefit plans we administer for our clients are uniquely designed specifically for that client. Since we don't do "one size fits all," most of our forms and materials are specific, not generic. The forms and brochures posted here are generic. You will need to contact your employer to obtain the enrollment and change forms specific to your benefit program. Your employer has provided you with a copy of the Summary Plan Description (SPD). Please refer to your SPD for all eligibility, enrollment, claims filing deadline and other specific issues.

We encourage you to log into your account on MyFlexOnline.com to create your claim for the fastest possible service. Claims created on the website and received by fax, mail or our new secured web claims portal by 9am Pacific on regular business days are processed same-day for the fastest possible reimbursement.
